v12.8.2 Update 1
If you open Outlook Web Access (OWA) through an Access Portal reverse proxy, when you log out from Outlook Web Access, you see a 403 Forbidden error page in your web browser. If you try to log in to Outlook Web Access through the Access Portal again, you continue to see the 403 Forbidden error, and cannot see the Outlook Web Access application page. To resolve the error, you must use your web browser settings to delete cookies. The problem seems to occur when the Outlook Web Access login uses the format domain\username.
Use a new incognito browser window each time you log in to Outlook Web Access. Because incognito mode does not permanently store cookies, you do not have to clear browser cookies each time you log in.
